Jurisdiction Contact Change Request 

The form on this page enables local jurisdictions and counties to provide the California Integrated Waste Management Board (CIWMB) with changes to contact information. This form is a temporary tool to streamline the process of keeping your contact information up to date. The CIWMB is developing a new data tool that will, in the future, enable local jurisdiction and county staff to manage their own contact information via a secure web interface.

Before You Start

Review your jurisdiction's contact information on the Local Assistance and Market Development Contacts page. Check for details such as the number of secondary jurisdiction contacts (only three are allowed) and the complete name of the jurisdiction (for example, either "Alameda" or "Alameda Unincorporated"). Confirming your jurisdiction's contact details ahead of time helps to ensure that your changes are made accurately and completely the first time. This effort will enable us to assist you efficiently as well as post your revisions quickly.

Information on Contact Roles

  • Primary Official Jurisdiction Contact: Receives the official notification of Integrated Waste Management Act (AB 939) actions, including annual report passwords and compliance notification. Each jurisdiction can have only one primary official contact.
  • Secondary Official Jurisdiction Contact: Includes jurisdiction staff who want to receive general information. Each jurisdiction can have up to three (3) secondary official contacts.
  • Annual Report Jurisdiction Contact: Receives information about the jurisdiction’s annual report. Each jurisdiction can have only one annual report contact. Note: The annual report jurisdiction contact should coordinate with the primary official jurisdiction contact about the annual report password.
  • Disposal Reporting System (DRS) Contact: The one person at the county and/or Regional Agency, who is assigned to report the county's DRS information to the CIWMB. Each county can have only one DRS contact.

Change Request Form

  • You must complete the fields marked with a red asterisk (*). Depending on the action requested, you may also need to complete other fields.
  • You must submit one change request form for each contact change.
  • You can easily submit multiple requests for the same jurisdiction. After submitting the first request, select your browser's back button to return to the form with the information you previously entered and then revise applicable fields. You may also select the "Clear Form" button to empty all form fields, if you prefer. Remember to select the "Submit" button upon completing the form to conclude the process for each request.
